Buttercross and Ilchester GP practices and Yeovil Health Centre, become the first to integrate into the new operating company: Symphony Healthcare Services
One year after achieving vanguard status, a further £4.9m funding is secured from NHS England.
September to December 2015: Design of joint venture to hold the single budget, and operating company to enable GP practices to integrate into a new organisation: Symphony Health Care Services.
GPs join the complex care team and the full model begins to operate.
April 2015 – September 2015: Enhanced primary care design expands the range of support available in GP practices. The first five practices commence the new method of working as they appoint the new Health Coaches.
A Programme Board is established between primary care and Yeovil Hospital. The full programme is established.
Symphony is chosen to be one of 29 new care model vanguard sites by NHS England, and awarded £4.9m funding.
The first complex care hub is set up in Yeovil District Hospital, with keyworkers and care co-ordinators and the first extensivist GP
The Primary Care Collaboration Fund (a national fund made available for primary care collaboration) awards South Somerset £200,000 to set up the first complex care hub.
July 2014–November 2014: Development of Test and Learn pilots by Somerset CCG to commission the new complex care model across Somerset.
December 2013 – June 2014: The complex care model is designed, based on delivering these outcomes.
75 patients with three or more conditions, carers and staff attend an event to share their experiences of the current system and their aspirations for change; less duplication and more co-ordination. A set of patient outcomes is developed.
Research by the Centre for Health Economics at York University finds that just four per cent of the population – those with multiple and complex conditions – account for 50 per cent of health and social care spend. The Symphony Project team find that those with three or more conditions are most likely to benefit from an integrated care.
What’s the real cost of health and social care in south Somerset? The South West Commissioning Support Unit is asked to create an anonymised dataset to examine the activity and costs across the patch.
Summer 2012. “There must be a better way”. Conversations start with staff from across the system and international care models are researched. Soon the Symphony Project Board is formed and a launch event held.
