Second Clinical Cabinet Date Announced – 14 September 2016
After the success of the first Clinical Cabinet (was Forum) in April this year, we are pleased to confirm the date and agenda for the next event on Wednesday 14 September 2016 between 9 and 12.30. With registration from 9 am at Yeovil Town Football Club.
This event is a unique opportunity to bring together Primary Care GPs and Hospital Consultants / Nurse Specialists to help to design and plan new ways of delivering healthcare across south Somerset. By bringing together Primary and Acute care services; the Symphony Programme aims to change the way that patients access and use healthcare services – focussing on the ‘right care’ in the ‘right place’ for the patient.
New specialist services and support roles in the community are now beginning to offer patients more choice and specialist treatment at local GP Practices, avoiding unnecessary admission tor readmission to Hospital.
The next Clinical Cabinet will cover the following topics:
- The development of the Sustainability and Transformational Plan and what the future of Healthcare could look like for Primary and Acute Care in south Somerset.
- How new models of care and roles such as Health Coaches are having a positive impact on both GPs and patients
- Introducing the latest updates on specialist nursing and education services in the community
- How to further develop the future model for delivering the ‘right care’ in the ‘right place’ for more patients across south Somerset
The event will conclude with a networking lunch between 12 and 12.30
